Service Description

An evaluation will consist of an initial overall assessment and review of a medications, general health, safety and education.  A care plan for each patient can then be customized to achieve ones' personal goals. We consistently draw on our breadth and depth of industry knowledge to provide optimal patient care, which is supported and consistently improved by tracking and measuring our patient outcomes. Our fully-licensed healthcare workers deliver high-quality care by implementing proven clinical protocols supplemented by the compassion each patient deserves. In addition, we implement clinical programs aimed at reducing re-hospitalizations and improve functional outcomes.
