Service Description

An evaluation will consist of initial assessment of ADL's and safety. Based on the observations, rehabilitation goals for each patient can be set. We consistently draw on our breadth and depth of industry knowledge to provide optimal patient care, which is supported and consistently improved by tracking and measuring our patient outcomes. Our fully-licensed healthcare workers deliver high-quality care by implementing proven clinical protocols supplemented by the compassion each patient deserves. In addition, we implement clinical programs aimed at reducing re-hospitalizations and improve functional outcomes .
